TL;dr: If HGH has high purity but IGF-1 blood levels are significantly too low, the manufacturing of the HGH hasn't been done correctly and that means the HGH is bad. I won't explain the whole manufacturing process of HGH because then the post will be even longer than it's gonna be, but: When a substance is tested by, for example Janoshik, it tests for the molecular mass (the weight of the HGH molecule). If practically all the powder in the vial has the correct weight, it will count as a 'success'; you can get 100% purity. However, that does not mean that the 3D structure of the substance, HGH in this instance, is correct. HGH is a protein and has a specific 3D structure. If it's not folded correctly, it has the same mass as HGH and the same atoms etc, but it's biologically inactive: misfolded HGH. It won't bind to the GH receptor, and won't tell your liver to make IGF-1. Cheap, high-volume recombinant protein production in Chinese biotech factories using E. coli fermentation systems (that bacteria is used for the synthesis of HGH), without strict control over folding, purification, lyophilization, or storage (such as wrong pH) can cause incorrect HGH to form.
